Below, we’ve also included some important information about our new patient forms, dental insurance, and financing options, as well as what you can expect during your first visit. Your First Visit
As soon as you call us to schedule your first appointment, one of our front desk staff will take your information over the phone and send you your new patient forms to fill out. On the day of your appointment, our team will greet you in the lobby and take you back to one of our comfortable operatories. You’ll see Dr. Mularczyk and one of our experienced hygienists for your checkup and professional dental cleaning. Before you leave, you’ll receive one of our new patient gift bags as well as a personal note from us to thank you for choosing our family-owned practice as your new dental home!
Patient Forms
Once your first appointment is scheduled with us, our front desk staff will send you our new patient forms. Or, you can download them here using the link below. In any case, we ask that you please fill these out and submit them to our team before your appointment to streamline your visit.
Dental Insurance
We proudly accept most PPO dental insurance plans and are in-network with Delta Dental Premier. Our goal is to maximize your benefits and help you save from steep out-of-pocket costs, which is why we’re happy to file insurance claims on your behalf no matter what PPO plan you have. Our team is also happy to answer any questions you may have about your dental insurance and help you navigate your benefits.
CareCredit Financing
In addition to helping our patients save by filling dental insurance claims, we also offer additional third-party financing through CareCredit. CareCredit specializes in offering financing for medical and dental procedures, allowing patients who qualify to choose from 6 or 12-month payment plans. This will allow you to spread larger out-of-pocket expenses over time, helping you more easily afford your procedure by allowing you to make smaller monthly payments.
